Allstate Whole Life Insurance w/Long-Term Care and Allstate Accident with Short-Term Disability

The Allstate Accident with Disability Income Rider is being offered to members to supplement your income during a disability. The Allstate plan covers a member both on and off-the-job.
Below are 3 income bars, illustrating the current weekly $300 Sickness and Health benefits, $500 Union Paid Lincoln Benefits and Allstate buy-up amounts based on income.
Please note: The Allstate Accident with the Disability Income Rider covers on and off-the-job sicknesses and accidents. The Allstate option pays 50% of the monthly benefit when a member is receiving workers compensation.

• MAXIMUM MONTHLY BENEFIT: $1,200 per month
• COVERAGES: On-the- Job Accident, Off-the Job Accident, Sickness Disability includes any non-accident
related surgeries.)
• DISABILITY BENEFIT DURATION: up to 6 months.
• ELIMINATION PERIOD: 3 days for Accident and Sickness.
• Elimination Period is the total number of days that you must be continuously disabled before your benefit
begins. Benefits are not payable during the elimination period.

PLAN LIMITATIONS:
• Pays 50% of the Monthly Benefit when you are receiving Workers Compensation.
• Medical questions are required for coverage.
• Monthly Benefit amounts are based upon 60% of your average monthly salary.
• PRE-EXISTING CONDITION: Benefits are not paid on loses occurring during the first 12 months of coverage if caused by a pre-existing condition. A pre-existing condition is a disease or physical condition for which symptoms existed within the 12 month period prior to the effective date.
• NECK & BACK LIMITATION: Insured is limited to only 3 months of disability benefit.
• Coverage available up to age 70 for active employees.
